When one of the children needs to see a doctor for any reason, they must be taken to the provinsial hospitals / free clinics. The house mother will then leave the rest of the children under supervision (which is sometimes impossible to organize) as she takes the sick child to a free medical facility (which sometimes takes almost a day).
